This is a listing of tools for integrating COVID-related care and immunization practices into your office via your EMR. This compendium will house a collection of vendor and community-driven solutions that work for primary care. As is always the case we rely on the contributions of the community at large to add new resources as they come online or flag resources that are no longer current. EMR-eReport Patient Vaccination Reconciliation Toolkit

These tools and toolkits were designed to be used with the Ontario Health’s eReport Practice Level COVID-19 Vaccination Report, also known as the COVaxON Aggregate Primary Care Vaccination Report, and requires access to Microsoft Excel.

Please note that Ontario Health's COVaxON Aggregate Primary Care Vaccination Report ("eReport") is not intended to be imported into your EMR. Each toolkit below helps reconcile a list of patients extracted from your EMR with the eReport outside your EMR which identifies your patients who have received a COVID-19 vaccine.

You can log in to access the report on the Ontario Health portal. Please note, a ONE® ID is required to access the COVaxON Aggregate Primary Care Vaccination Report.
