Consent

PrimaryCareNetwork-mh.ca respects your right to privacy. We will not collect, use, or disclose your personal information without your prior consent unless otherwise required by law. When you make use of this website your consent may be expressed or implied. The Mississauga Health Primary Care Network (MH PCN) will seek your consent to use or disclose the information when we collect it. The form of consent sought by the MH PCN may vary, depending upon the circumstances and type of information disclosed. In determining the appropriate form of consent, we shall take into account the sensitivity of the personal information provided. The MH PCN will seek your express consent when the information is likely to be considered sensitive. Implied consent will generally be appropriate where the information is less sensitive.

You may withdraw your consent at any time. In order to withdraw consent, you must provide notice to the PrimaryCareNetwork-mh.ca in writing. Methods to covey these instructions include emailing our admin team at MHPrimaryCareNetwork@gmail.com. Withdrawing consent through a partner organization does not guarantee that the PrimaryCareNetwork-mh.ca will be notified of your withdrawal.

Limiting Use, Disclosure and Retention

We do not sell the personal information we collect. We do not share the personal information we collect without a user’s permission. We will not disclose your personal information to anyone else without your prior knowledge or consent, except when required by a government body or agency, or as permitted by law. The MH PCN will not use, disclose, or transfer your personal information for any purpose without first obtaining your consent, where such consent is not reasonably implied.

How We Use Aggregated or Anonymized Information

In an ongoing effort to better understand your needs so as to improve the services we provide, we may use aggregated or anonymized information to analyze user demographics. This analysis may be used by the MH PCN and any partners that embed MH PCN content for statistical purposes and to develop and enhance MH PCN activities and services.
