In follow-up to our meeting last Thursday please find the invitation below or in the attachment for the engagement sessions.
The Mississauga Ontario Health Team (MOHT), in partnership with the Mississauga Health Primary Care Network (PCN) and Trillium Health Partners (THP), is excited to share that we will be launching SCOPE (Seamless Care Optimizing the Patient Experience) to connect primary care providers to a virtual interprofessional team improving access to care for patients, families and caregivers.
To ensure SCOPE’s success, we would like to invite you to participate in one of four SCOPE Town Hall Engagement Sessions to share more information, answer your questions, and most importantly hear from you which SCOPE services would most enhance your practice.
