Peel Region Paramedic Services have raised that they are still receiving at-home swab requests from primary care physicians and other community providers at their old fax number.

Please note that the Region of Peel's Paramedic Services COVID-19 At-Home Swab Request has been changed to 905-564-9511.

Instructions:

Complete the PRPS At-Home Swab Request Form + PHO Lab Requisition and fax both forms to 905-564-9511.
